I hereby request and accept membership in Local 338, Retail, Wholesale, and Department Store Union, UFCW, (hereafter “Union”) and agree to be bound by its Constitution, Bylaws and the amendments thereto now in effect or subsequently enacted by the Union. I understand that the Union’s Constitution and/or Bylaws requires me to pay dues to maintain continuity of membership. My membership in the Union shall be continuous unless I notify Local 338’s Secretary Treasurer in writing that I intend to resign.
I hereby assign to LOCAL 338, RWDSU, UFCW, (hereafter “Union”) from my compensation the Union's current dues amount and any duly authorized dues increase, to be deducted in each regular payroll from my salary/wages commencing with the next regular payroll period. This authorization and assignment is voluntarily made in consideration for the cost of representation and collective bargaining and is not contingent upon my present or future membership in the Union. I therefore authorize my Employer to deduct from my salary/wages all Union dues and other fees or assessments as shall be certified by the Secretary-Treasurer of the Union under its Constitution and Bylaws and to remit those amounts to Local 338, RWDSU, UFCW.
This assignment and authorization shall remain in effect and be irrevocable, regardless of whether I am or remain a member of the Union, for the period of one year from the date of my execution of this authorization or until the termination date of the collective bargaining agreement between the Employer and Union, whichever occurs sooner, and shall remain in full force and effect from year to year thereafter, unless I revoke it by sending written notice of my desire to revoke this authorization to the Secretary-Treasurer of Local 338, RWDSU, UFCW, and to the Employer, postmarked or received by both not less than 30 days and not more than 60 days prior to the end of any subsequent yearly period.
In addition, the Secretary-Treasurer of the Union is authorized to deposit this authorization with any employer that has a collective bargaining agreement with the Union and is further authorized to transfer this authorization to any other employer with a collective bargaining agreement with the Union in the event that I should change employment.
Further, this authorization shall remain in effect in the event that there is any break in employment with the Employer or any subsequent employer as described in the preceding paragraph, including but not limited to: a layoff, leave of absence, or termination from employment. In the event that I resume employment with the Employer or any subsequent employer after any break in employment, I authorize deductions to resume as of the next regular payroll period upon the resumption of employment with the Employer or any subsequent employer.
Should any provision of this assignment and authorization be adjudged to be unenforceable, the invalidated provision shall be enforced to the full extent of the law and shall not affect the validity of the remainder of this authorization.
Effective with my next pay period, I hereby authorize and direct my Employer to deduct from my salary the sum indicated below each week and remit it to the RWDSU, Local 338 Political Action Committee. By authorizing this deduction, I understand that this contribution is voluntary and that I may refuse to contribute; that I am a U.S. Citizen or Legal Permanent Resident; and that this contribution is not deductible on my federal income taxes.
By signing below, I grant to Local 338 RWDSU the right to use, reproduce, and publish photographs, videos and/or audio recordings of my image, likeness, and voice without payment, for publicity, illustration, education, or any other lawful purpose and in any manner and medium, including, but not limited to, print or electronic material, with or without my name; and to alter and composite my image, likeness and/or voice without restriction. I understand that this authorization is continuous and I forever release Local 338 RWDSU, the PAC, and their legal representatives and assigns for any and all claims and liability in relation to the use of any photographs, videos and/or audio recordings, including, without limitation, any claims of privacy.
You have the right not to be a union member. Non-members have the right to object to paying for union activities not related to the union’s duties as bargaining agent and to obtain a reduction in fees for such activities. You have the right to receive sufficient information to enable you to intelligently decide whether to object, and the right to be told of the union’s internal procedures for becoming an objector.
